If you either toss them too high initially, this problem can happen. If you also hammer them while they have enough velocity upwards, this can also happen. You need to find a sweet-spot with either your timing or the strength of the initial toss so that you can still hook safely straight ahead whilst performing the save. It really is just a practise makes perfect scenario. But at the start when learning this, you might want to re-arrange your hook slightly away after a save just for safety reasons, but eventually you can and should be able to hook at the same place before and after the save.
