If you turn on cl_dummy_control, you can use cl_dummy_hook

For those dummy control binds I like to set them to work the same way as deep fly, so when you activate it, mouse1 will fire pistol + dummy hook

Check out the "using 2 config files" section on the binds wiki linked above. It's very similar to what you want, basically swap mouse1 with mouse2 and +fire with +hook

is there any way to fix the weird ping spikes that happens every few seconds? in servers i have normaly 30-60 ping but then suddendly a lag appears and ruins my run

Use LAN cable instead of Wifi, disable power saving in your network adapter settings, turn off other programs running, turn off other devices in your network
